Sometimes they send out a warning, and sometimes they simply send out a termination email.
If they send you a warning first, be thankful. That means that
- your "infraction" may be minor -- e.g. you put Adsense in a new site that is borderline acceptable to Adsense
- they see value in your site -- e.g. they think you have the potential to really earn more from the program
- you have already given them significant income in the past and would like to continue the relationship but you need to clean up your act first
But if they send you an outright termination, it means that either your "violation" is severe, or they don't seen any potential and value in your websites with the Adsense program
Remember if you receive your termination email, you can still appeal the termination. I know of several website owners who successfully overturned the termination after presenting solid evidence that they did not generate the invalid clicks. Use this appeals form

I think it would depend on the violation.
Certain things are more serious than others. Some you might get a warning, others you may be terminated immediately.
If you get caught violating, they send you a note indicating the violation and they pause your active buy until the violation is cleared.
